Program
Overview:
The Title XIX HCB MR/DD Waiver Program is a Federal
and State funded Medicaid program designed to deliver services to
individuals in their home and community surroundings as an
alternative to receiving services in an ICF/MR facility. The
program serves persons with mental retardation and other
related conditions. We at Seneca Health Services, Inc. strive to
provide services as described within the context of an
Individualized Treatment Plan using a person centered approach that will
empower individuals who have mental retardation or a related condition
by providing support services designed to achieve the highest level of
independence and self-sufficiency possible within their homes and communities.
Services Available:
Habilitation Service Coordination, Respite Care, Agency Residential, Community
Residential Habilitation, Residential Habilitation Special Projects, Adult
Companion, QMRP services, Nursing services, Day Habilitation, Supported
Employment, Day Hab Transportation and Environmental Accessibility Adaptations.
Eligibility
Criteria:
Financial Eligibility is determined by the County DHHR
Office of the
person’s residence.
- Income: The individual’s
monthly income may not exceed 300% of the current maximum Supplemental
Security Income (SSI).The parent’s income is not considered in
determining financial eligibility. An individual does not have to be
SSI eligible to become eligible for the MR/DD Waiver Program.
- Assets: An individual’s
assets may not exceed $2,000
Parents assets are not considered when determining eligibility.
Medical Eligibility:
Both a diagnosis of mental retardation and/or a related
condition and the need for ICF/MR Level Care are required.
Evaluations used to make this determination include an Annual
Physical (DD-2A), a Psychological Evaluation (DD-3), and a
Social History.
Program eligibility
is determined by the Office of Behavioral Health
Services and the Bureau for Medical Services.
