GROUP HEALTH INSURANCE Eligible employees have the option of enrolling in one of two health plans offered by Seneca, both of which include a prescription drug plan. The health coverage is with Public Employees Insurance Agency (PEIA) and administered by Wells Fargo. The prescription benefits are administered by Express-Scripts. Seneca pays 75% of the premium for single and family coverage. Care provided by participating network physicians is generally covered at 80% and care provided by non-participating physicians is covered at 60%, unless prior authorized. The PEIA preferred provider organization (PPO) is a network of physicians and hospitals from whom PEIA members can receive care at the highest level of benefit. This network consists of WV physicians and hospitals who accept PEIA’s reimbursement, and out-of-state physicians and hospitals who participate in PEIA’s out-of-state networks. To view a listing of participating pharmacies, press the Express-Scripts web address on the “LINKS” page. DENTAL/VISION INSURANCE Eligible employees may elect dental/vision coverage. There is a $50 deductible for basic and major dental services and the plan pays a set percentage or dollar amount for covered services. Seneca pays 50% of the premium for single coverage. Family coverage is available at the employees’ expense. LIFE INSURANCE & DISABILITY BENEFITS Term Life and Accidental Death & Dismemberment (AD&D) Insurance. Seneca pays 100% of the premium. The benefit amount is 200% of the covered employee’s salary up to a maximum of $200,000 plus $10,000. Coverage for spouse is $5,000 and $1,000 for dependent children. Optional Life Insurance. Eligible employees may elect
additional life insurance at their own expense. Benefit amounts available
for employees up to $500,000. For spouse amounts available are $5,000,
$10,000, $15,000 or $20,000 and for dependent children Long Term Disability (LTD). Seneca pays 100% of the premium. The benefit amount is 60% of monthly pay subject to a maximum of $5,000/month with a qualifying period of six months. 403(b) RETIREMENT PLANSeneca contributes 5% of an eligible employee’s monthly gross pay, less any overtime pay. Employees may contribute 1% to 15% up to a a calendar year maximum in accordance with IRS and/or ERISA provisions. Participants are fully vested. SECTION 125 AND SUPPLEMENTAL INSURANCE Section 125 Premium Only Plan allows employees to pay for health and other insurance premiums with before tax dollars. Supplemental coverage for cancer, heart disease, accident, and short-term disability available through Colonial Life Insurance Company. LEAVE Seneca provides generous
paid leave time, including accrued vacation, accrued sick, holidays and
bereavement leave. |
